Permission to use photographs and film footage

 

I agree to participating in a photo shoot and/or filming for The Bristol Rainforest (the Producer).

The details of the photo/video shoot are below:

Location: Bristol

 

Production name: Various different productions.

 

I acknowledge that by signing this release form and attending the above detailed shoot, I agree to The Bristol Rainforest (the Producer) to record and edit into the Production and related materials my name, likeness, image, voice and performance on the Recordings.

 

I understand that my participation in this project is voluntary and that I may withdraw my consent at any time prior to, or after, the photography, filming or recording.

 

I agree that the Recordings may be edited and otherwise altered at the discretion of the Producer and used in whole or in part for any and all broadcast, non-broadcast, audio/visual, and/or exhibition purposes in any manner or media throughout the world. This includes materials produced for fundraising purposes. But any substantial changes to my likeness will need my consent.

 

I agree that the Producer may use and authorise others to use all or parts of the Production and that the Producer, its successors and assigns shall own all rights, title and interest, including copyright to the Recordings captured for the Production, to be used and disposed of without limitation.

 

In signing this release, I agree that the Producer may store copies of the Recordings created and that they may store the contact details I have provided alongside this imagery in its database, and consent to be contacted regarding the Recordings in the future. These contact details will not be shared with any third parties for other uses.

 

If the Recordings are kept for longer than 5 years, the Producer will need to seek my consent again to use it.
